You can create checkout questions for your customers to answer during their checkout process. Add checkout attributes by following these steps.

 

Go to Inventory => Options => Checkout Attributes

 

These questions can be used to gather information about your customers, or perhaps to gauge their overall experience with your site. For example, one of your questions may be a simple "How did you hear about us?" type of query.
